Calibre is a totally free PDF to ePub Converter which can convert PDF to ePub files easily. It supports:

Input Formats: CBZ, CBR, CBC, CHM, EPUB, FB2, HTML, LIT, LRF, MOBI, ODT, PDF, PRC**, PDB, PML, RB, RTF, SNB, TCR, TXT
Output Formats: EPUB, FB2, OEB, LIT, LRF, MOBI, PDB, PML, RB, PDF, SNB, TCR, TXT
** PRC is a generic format, Calibre supports PRC files with TextRead and MOBIBook headers

Tips before using Calibre:

PDF to ePub Converter Freeware User Guide

Step 1: The first thing you'll need to do is enter your settings for Look & Feel.

Step 2: Second is your Page Setup. This is where you select your device and set your page margins.

Step 3: Third is Structure Detection which is where you enter your Table of Contents settings.

Step 4: Add PDF to Calibre. Then highlight the story and click the button Convert E-Books:

You'll get a box like this and this is where you'd add the author name, title, novel summary, book cover, and etc...

Step 5: Once you hit OK, Calibre will convert your ebook for you.
